Students remain responsible for payment. <strong>Do NOT file this form if you are appealing for:<\/strong><\/p>\n<ul>\n<li><strong>Late Registration Fee<\/strong>\u00a0\u2013 non-refundable per the University\u2019s Tuition and Fee Manual<\/li>\n<li><strong>Change of Program Fee<\/strong>\u00a0\u2013 non-refundable per the University\u2019s Tuition and Fee Manual<\/li>\n<li><strong>Inability to pay your bill<\/strong>\u00a0\u2013 contact the Bursar\u2019s Office to explore your options<\/li>\n<li><strong>Drop penalty fee<\/strong>\u00a0\u2013 incurred for dropping classes on or after the first official day of the term<\/li>\n<li><strong>Enrollment at a<\/strong>\u00a0<strong>non-CUNY college<\/strong> for the semester in question &#8211; not appealable &#8211; unwanted courses must be dropped before the term begins to avoid charges. Drops on or after the term start date incur penalties in alignment with the college&#8217;s <a href=\"https:\/\/www.qc.cuny.edu\/br\/\" target=\"_blank\" rel=\"noopener\">Tuition and Fee Liability Schedule<\/a>.\u00a0<\/li>\n<li><strong>WN grade or another grade received without class attendance<\/strong> \u2013 not appealable unless extenuating circumstances are documented<\/li>\n<li><strong>Grade change request<\/strong>\u00a0\u2013 email\u00a0<a href=\"mailto:student.advocacy@qc.cuny.edu\">student.advocacy@qc.cuny.edu<\/a><\/li>\n<\/ul>\n<p>[\/et_pb_text][et_pb_text ul_item_indent=&#8221;30px&#8221; _builder_version=&#8221;4.27.4&#8243; _module_preset=&#8221;default&#8221; text_font_size=&#8221;16px&#8221; ul_line_height=&#8221;1.5em&#8221; header_3_font=&#8221;Open Sans|700|||on||||&#8221; header_3_text_color=&#8221;#000000&#8243; header_3_font_size=&#8221;18px&#8221; header_4_font=&#8221;Open Sans|600|||on||||&#8221; header_4_text_color=&#8221;#000000&#8243; global_colors_info=&#8221;{}&#8221;]<\/p>\n<h3>Valid Grounds for Filing an Appeal (with Required Official Documentation)<\/h3>\n<ul class=\"no-bottom-margin-list\">\n<li><strong>Death of the student or immediate family member<\/strong> (parent, child, spouse, domestic partner, sibling, or grandparent):\n<ul class=\"no-bottom-margin-list\">\n<li>Submit: copy of the death certificate, obituary, or death notice clearly showing the relationship of the deceased to the student.<\/li>\n<\/ul>\n<\/li>\n<li><strong>Call to active military duty<\/strong>:\n<ul class=\"no-bottom-margin-list\">\n<li>Submit: official deployment\/reactivation notice with dates within the appealed semester.<\/li>\n<\/ul>\n<\/li>\n<li><strong>Accident preventing continued attendance<\/strong>:\n<ul>\n<li>Submit: \u00a0accident report and hospital discharge papers.<\/li>\n<\/ul>\n<\/li>\n<li><strong>Hospitalization<\/strong>:\n<ul>\n<li>Submit: hospital discharge papers.<\/li>\n<\/ul>\n<\/li>\n<li><strong>Illness or injury of an immediate family member for whom you were the primary caregiver<\/strong>:\n<ul class=\"no-bottom-margin-list\">\n<li>Submit: signed letter from the family member\u2019s medical provider, on official letterhead, stating the dates of illness and naming you as the primary caregiver.<\/li>\n<li>Medical details do not need to be disclosed.
