Students need to submit the <a href=\"https:\/\/www.qc.cuny.edu\/health\/wp-content\/uploads\/sites\/71\/2021\/02\/QC-ImmRecordForm-0918.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">immunization record form<\/a> before registering for classes. All students (regardless of age) must complete the <a href=\"https:\/\/www.qc.cuny.edu\/health\/wp-content\/uploads\/sites\/71\/2021\/02\/QC-ImmRecordForm-0918.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">meningitis response form.<\/a><\/p>\n<p>&nbsp;<br \/>\n[\/et_pb_text][et_pb_accordion open_toggle_text_color=&#8221;#E71939&#8243; open_toggle_background_color=&#8221;#ffffff&#8221; closed_toggle_background_color=&#8221;#ffffff&#8221; icon_color=&#8221;#e71939&#8243; _builder_version=&#8221;4.27.4&#8243; _module_preset=&#8221;default&#8221; toggle_text_color=&#8221;#E71939&#8243; toggle_font=&#8221;Open Sans|600|||||||&#8221; body_font=&#8221;Open Sans||||||||&#8221; body_text_color=&#8221;#000000&#8243; body_font_size=&#8221;16px&#8221; hover_enabled=&#8221;0&#8243; border_color_all=&#8221;#000000&#8243; global_colors_info=&#8221;{}&#8221; sticky_enabled=&#8221;0&#8243;][et_pb_accordion_item title=&#8221;Measles, Mumps, and Rubella Requirements&#8221; _builder_version=&#8221;4.16&#8243; _module_preset=&#8221;default&#8221; global_colors_info=&#8221;{}&#8221; open=&#8221;off&#8221;]In order to register for classes, students born after December 31, 1956, have to provide proof of the following:<\/p>\n<ul>\n<li><strong>Two<\/strong> measles vaccines given after 1968; on or after your first birthday; and at least 28 days apart.<\/li>\n<li><strong>One<\/strong> mumps vaccine given on or after your first birthday and dates 1969 or later.<\/li>\n<li><strong>One<\/strong> rubella vaccine given on or after your first birthday and dates 1969 or later.<\/li>\n<\/ul>\n<p style=\"text-align: center\"><strong>OR<\/strong><\/p>\n<ul>\n<li><strong>Two<\/strong> MMR vaccines given after 1972; on or after your first birthday; and at least 28 days apart.<\/li>\n<\/ul>\n<p style=\"text-align: center\"><strong>OR<\/strong><\/p>\n<ul>\n<li>Immunity to measles, mumps, and rubella as documented by blood test (MMR titer). Original lab report <strong>MUST<\/strong> be submitted to the Health Services Center.<\/li>\n<\/ul>\n<p>[\/et_pb_accordion_item][et_pb_accordion_item title=&#8221;Acceptable Proof of Immunity&#8221; _builder_version=&#8221;4.16&#8243; _module_preset=&#8221;default&#8221; global_colors_info=&#8221;{}&#8221; open=&#8221;off&#8221;]<\/p>\n<ul>\n<li>Childhood immunization card signed and stamped by the medical providers<\/li>\n<li>Immunization records from your doctor\u2019s office (signed and stamped by doctor or clinic)<\/li>\n<li>High school or prior college\u2019s immunization records<\/li>\n<li>Blood test, including original lab report, proving immunity to measles, mumps, and rubella (MMR titer)<\/li>\n<li>Local health department or online city registry<\/li>\n<\/ul>\n<p><strong>Records must<\/strong><\/p>\n<ul>\n<li>Clearly indicate the type of vaccine, dates of vaccine, and name and address of the doctor or clinic<\/li>\n<li>Be stamped and signed by the doctor or clinic<\/li>\n<\/ul>\n<p>MMR vaccination is available at the Student Health Center free of charge (for ºÚÁÏרÇø students only). Contact our office and make an appointment with our nurse. \u200b<br \/>\n[\/et_pb_accordion_item][et_pb_accordion_item title=&#8221;Meningitis Requirements&#8221; open=&#8221;on&#8221; _builder_version=&#8221;4.27.4&#8243; _module_preset=&#8221;default&#8221; link_option_url_new_window=&#8221;on&#8221; hover_enabled=&#8221;0&#8243; global_colors_info=&#8221;{}&#8221; sticky_enabled=&#8221;0&#8243;]<\/p>\n<p style=\"text-align: center\">New York State Public Health Law 2167 requires all colleges to provide information on <a href=\"https:\/\/www.cdc.gov\/meningococcal\/index.html\" target=\"_blank\" rel=\"noopener\">meningitis<\/a> and the meningitis vaccine. ALL STUDENTS (<strong>regardless of age<\/strong>) must complete a <a href=\"https:\/\/www.qc.cuny.edu\/health\/wp-content\/uploads\/sites\/71\/2021\/02\/QC-ImmRecordForm-0918.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">meningitis response form<\/a> in which students must respond to one of the following:<\/p>\n<ol>\n<li style=\"text-align: center\"><span class=\"TextRun SCXW162670603 BCX0\" lang=\"EN-US\" xml:lang=\"EN-US\" data-contrast=\"none\"><span class=\"NormalTextRun ContextualSpellingAndGrammarErrorV2Themed SCXW162670603 BCX0\">Had<\/span><span class=\"NormalTextRun SCXW162670603 BCX0\"> meningococcal immunization within the past 5 years. The vaccine record is attached.
